1. Sample submission requirements

- Contact us prior to sending your samples. The earlier you contact us the better, as it will help us to plan our testing service schedule, which in turn will provide a faster turnaround on your sample testing. No sample shipments are accepted without prior contact.

- Make payment arrangements with us. We require a purchase order, credit card, or other payment arrangements before we will report results.

- Label samples to be tested either with a label provided by Salimetrics or use a laser printed label of your own. Also, it is advisable to write the subject number on the vial using permanent ink (Sharpie-brand marker) in case your label accidentally falls off.  

-   Create a sample roster listing the identifying information for each vial included in the shipment. We prefer that you create an Excel file and e-mail it to us as an attachment.

- If our example is followed, we will be able to insert your results directly into the spreadsheet and e-mail it back to you. If you cannot provide your sample information electronically, we require that your sample labels include unique numbers logically sequenced no more than five (5) digits long.

- Complete the Testing Services Order Form .This form must be included with your samples.

- Package your samples and ship them according to our Sample Shipping Instructions , which follow Centers for Disease Control and Prevention regulations on interstate shipping of etiological agents. We recommend that you contact us one business day before samples are sent. We do not accept SATURDAY or SUNDAY deliveries. We recommend shipping samples Monday through Wednesday only.

- Upon delivery, your samples will be inventoried against the sample roster you provide. We will contact the person listed on the Testing Services Order Form to confirm receipt of your samples.

- Depending on the number of samples we already have in-house, testing will begin as soon as possible. In some cases, we may offer a Fast-Trac service that will offer a faster turnaround for an additional charge. Contact us for details on this service.

3. Creating a Sample Roster

·  Create a sample roster listing the identifying information (in ascending order) for each vial included in the shipment. We prefer that you create an Excel file and e-mail it to us as an attachment.

·  Please use one row per sample without empty rows, following this example:

·  If you are using multiple parameters for subject ID, please renumber the samples (both sample vial & on roster) using a simple numbering system before you ship the samples.

Note: You will be charged an additional fee if we are required to renumber and organize your samples once they arrive at Salimetrics.

·  If this example is followed, we will be able to insert your results directly into the spreadsheet and e-mail it back to you.

·  If you cannot provide your sample information electronically, we require that your sample labels include unique numbers logically sequenced no more than five (5) digits long and that you include a list of these numbers with the sample shipment.

4. Final Results Report

· You will receive a report only after testing is completed.

· Results are reported via e-mail in the form of an Excel file. If you prefer your results in a different format or sent by other means we will do our best to fulfill your request.

· The results will include two values for each sample and the mean in the form of a concentration (e.g., cortisol = 0.652 µg/dL).

· If you provide the sample roster in electronic form as described in Section 3 above, we will include this information in the final report.

· If you do not provide an electronic roster, the final results report will show only the five (5) digit sample ID.
 

· Assay performance data including coefficients of variation are available upon request.

5. Sample Storage Policy

We will store your samples for 30 days after testing at no charge. After 30 days we will proceed per your instructions on the Testing Services Order Form:

(1) Dispose of your samples (no charge)
(2) Send your samples back to you (you pay shipping costs)
(3) Store your samples in our freezers for a per sample charge
